Vaccines produced by Genetic Engineering

Recombinant DNA techniques are used to modify the genome of living things (plants, animals, microorganisms) in order to change their physical, physiological, metabolic characteristics. It is thus possible to use genetically modified microorganisms to produce biological substances for therapeutic use as well as vaccines. This technique has made it possible to manufacture vaccines with a chimeric genetic heritage by inserting genes, coding for proteins recognized by the immune system, in support microorganisms. Vaccines are thus produced for animals, for example against rabies, rinderpest, bovine leukosis, Newcastle disease of poultry. Such chimeric vaccines have also been designed for humans. A vaccine against rotaviruses, responsible for many diarrheas around the world, has been obtained by associating human genes with a simian strain of rotavirus. Put on the market in the United States, it was withdrawn in disaster because infants were suffering from diverticulosis. But the best known example of recombinant vaccines is that of hepatitis B vaccines (GenHevac B®, Engerix B®, HBVax DNA® or Recombivax®). The technique here consists in producing, by a cellular system, the surface protein of the hepatitis B virus (HBs) by means of a recombinant plasmid introduced into these cells.

GenHevac B® case

The recombinant plasmid (designated by pSVS dhfr) comprises:
- The support plasmid: Bolivar and Rodriguez plasmid (pBR322) originating from Escherichia coli and comprising its origin of replication and a gene for resistance to ampicillin,
- The hepatitis B virus S gene (with the pre-S2 sequence), placed under the control of the strong promoter of the simian virus SV40,
- The dhfr marker gene, allowing the synthesis of dihydrofolate reductase, placed under the control of the mouse mammary tumor virus (MMTV),
- This plasmid is then cloned into bacteria, the ampicillin resistance gene making it possible to sort out the bacteria that have incorporated the plasmids.


Transfection and expression of plasmids
The plasmids are then transfected into cells of continuous CHO dhfr- line (originating from Chinese hamster ovary and deficient in dihydrofolate reductase). After transfection, the cell cultures are treated with methotrexate. The cells which have incorporated the plasmid pSVS dhfr resist this treatment and can translate the gene of interest S.

Other hepatitis B vaccines

Engerix B® and HBvax DNA® (or Recombivax®) are produced on the same principle but with a few differences: absence of the pre-S2 sequence in the recombinant plasmid, use of yeasts in place of CHO cells (which has two consequences linked to the specific functional activity of yeasts: non-glycosylation of the HBs proteins obtained, need for grinding due to the absence of exocytosis of the products produced).

THE RISKS

The risks are linked to both the culture media and the techniques of genetic engineering and essentially concern in both cases the contamination of the final product with DNA.
This DNA is potentially carcinogenic:
- on the one hand because it can contain residues with carcinogenic consequences (it should be remembered that cells of continuous line type CHO have the characteristics of cancer cells: abnormal karyotype, loss of contact inhibition),
- on the other hand, and above all, because DNA can, by integrating into the genome of our cells, reactivate a proto-oncogene or inactivate an anti-oncogene. This risk is all the more worrying as in the DNA impurities can be found the viral sequences used for the preparation of the recombinant plasmid, in particular the promoters.
A group of experts convened by the WHO in 1986 had mentioned the risk of "malignancy which a heterogeneous contaminating DNA could present in the long term, in particular if it turns out to contain potentially oncogenic coding or regulatory sequences. "Because of these risks, the WHO has set the acceptable limit dose at 100 pg of DNA per purified dose, a limit raised in 1998 to 10 ng. Hepatitis B vaccines contain between 1 and 30 µg per dose. Does this mean that a quantity as small as a picogram is safe? Certainly not when we know that this represents a billion base pairs of DNA and that the size of a viral promoter does not exceed 20 to 40 base pairs.
The experts, authors of technical report 747 (1987) declared moreover very cautiously: "Several points must be taken into account when making these risk estimates:
First, all calculations are based on the assumption that the risk factor for tumor induction decreases linearly with the concentration of DNA. This assumption is not necessarily correct, since a quantity of DNA which has no measurable biological effect in a standardized test because it is present at too low a concentration, can nevertheless have an effect in certain conditions or on certain organs or tissues.
Second, it is not yet known whether the risk associated with repeated exposure to DNA will act cumulatively or not.
Third, consideration should be given to the possibility that DNA preparations which do not induce tumors in experimental systems may cause changes in humans which may increase the incidence of the development of tumors after long periods of latency.
Fourth, experiments conducted on short-lived animals do not allow an assessment of the long-term effects of acquired DNA sequences. "

Protein contamination
The operations of construction of the recombinant plasmids require the use of various enzymes among which the deoxynucleotidyl terminal transferase (dTT) capable of adding nucleotides to DNA without the presence of template and thus causing mutations. Engerix B® vaccines contained it. Do they still contain it? Hepatitis vaccines are more or less loaded with impurities (between 1 and 5%), probably protein for the most part, without anyone knowing what it is. Enzymes? Transforming proteins? Are all these molecules deactivated?

Disadvantages of co-amplification
After transfection, a co-amplification phenomenon occurs, between the S and dhfr genes, stimulated by treatments with increasing dose of methotrexate. Unfortunately, these amplification systems have a major drawback, that of causing genetic rearrangements (translocations, deletions). Some of these events can occur in the DNA sequence coding for the desired protein and lead to the production of an altered protein, potentially dangerous during therapeutic use. Could this be the beginning of an explanation for the numerous disorders observed in subjects vaccinated against hepatitis B? The disorders observed after vaccination against hepatitis B are quite comparable to the complications of hepatitis B itself. Accidents are very diverse and can affect most organs:
- in the first row, neurological accidents: multiple sclerosis, leukoencephalitis, transverse myelitis, acute cerebellar ataxia, polyneuropathy,

- eye accidents, in particular occlusions of the central retinal vein, - muscular accidents: macrophage myofasciitis (muscle and joint pain, intense fatigue),
- bone and joint accidents,
- dermatological accidents: lichen planus, various erythemas, alopecia,
- kidney accidents: glomerulonephritis,
- cardiovascular accidents: vasculitis, knotty periarteritis,
- blood accidents: thrombocytopenic purpura,
- autoimmune diseases: systemic lupus erythematosus, insulin-dependent diabetes.

Two causes can be at the origin of these accidents:
- The first is immunological.The hepatitis B virus is not cytopathogenic, the destruction of hepatocytes would result from the immune reaction and the complications of the disease would result from deposits of immune complexes triggering an inflammatory reaction and destruction tissues, where the deposit occurred. Large complexes are normally destroyed by macrophages, but if the affinity between antigen and antibody is low, the immune complexes are small, soluble, and eventually settle without being destroyed. A special case is that of macrophage myofasciitis, due to an accumulation, in the connective tissue of the injected muscle, of macrophages containing aluminum crystals (used as an adjuvant of the vaccine). It follows a permanent activation of the immune system.
- The second cause would be molecular mimicry between elements of the virus (or vaccine) and molecules of the organism, for example in demyelinating attacks. In addition, gamma interferon, produced during the immune reaction, causes the expression of HLA II molecules by cells which do not usually express them. These cells then become capable of presenting to the immune system unusual peptides which are no longer recognized as part of the self, hence their destruction. A mechanism of this type is probably the cause of insulin-dependent diabetes. Various HLA haplotypes predispose to autoimmune diseases: A1, B8, DR3 for autoimmunity in general, A3, B7, DR2 for multiple sclerosis, B8 and / or DR3 for impaired eliminating function of immune complexes by macrophages. These disorders have been regularly published, especially since the compulsory vaccination of medical personnel (1991) and the major vaccination campaigns of 1994-95, both in international journals (including The Lancet, New England Journal of Medicine, British Medical Journal , Science, Neurology, Journal of Hepatology, American Journal of Ophtalmology, Journal of Rheumatology, Clinical Nephrology, Nephron, American Journal of Epidemiology, Muscle and Nerve), only French (La Presse Médicale, Archives de Pédiatrie, Revue de Médecine interne). The precise quantitative evaluation of these accidents is difficult because pharmacovigilance only identifies a small part of them (between 1 and 10%). For example, in the "intensive" follow-up of the school vaccination campaign, 99% of high school students included in the surveillance program were lost to follow-up, only 100 out of 10 having been followed up after three years. Recombinant DNA techniques make it possible to manufacture numerous molecules for therapeutic use. If it is conceivable, despite the risks, to use them to treat patients especially when the life prognosis is at stake, is this also admissible for vaccines, injected into healthy subjects who may never contract the disease from which we want to protect them. There is a major information deficit in this area which does not allow citizens to make an informed and responsible free choice.
